Peer Health Navigator – My House

Nashville CARES 

Full time Hourly (Non-Exempt) \ 11/3/2020

Summary:  The Peer Health Navigator will mentor and provide HIV/STI prevention, HIV medical care,

HIV treatment adherence, and general GLBT health promotion support, navigation and life skills

based coaching for Young African American Men who have Sex with Men (YAAMSM) at My House and community locations to assist the client in pursuing their individual health and wellness journey.

 

Description of Duties & Responsibilities:  As a member of the GLBT services team, the Peer Health Navigator:

  • Provides individualized STI/HIV prevention support, information, training and passport certification;

  • Provides the EBI ARTAS to assure effective linkage to HIV medical care and/or effective supportive services for the prevention of HIV;

  • Teaches others about managing their mental and physical health, especially related to GLBT specific health activities, screenings, and medical and behavioral health care;

  • Supports clients in learning how to make good choices for themselves including development of short, medium and long term plans for health and success;

  • Creates and adapts wellness objectives to overcome barriers to good mental and physical health based on needs of, and with input from, clients;

  • Co-facilitates groups/classes to support improved health outcomes for “My House” clients;

  • Works closely with “My House” Inter-disciplinary teams (educators, case managers, other staff) to engage clients in appropriate services and resources and assist with coordination of care;

  • As relevant, promotes consumer engagement in therapy with clinicians, engagement with psychiatric appointments, compliance with the healthcare registry, involvement and engagement with wellness services and engagement with primary care, especially HIV primary care for YAAMSM living with HIV;

  • Completes all tracking and reporting requirements for outcomes and evaluation;

  • Maintains appropriate professional standards and provides appropriate follow-up for consumers;

  • Provides appropriate linkage and referral to relevant services to optimize health outcomes.

 

Administrative

  • Completes activity reports and documentation in accordance with department processes;

  • Reports on output and outcomes, as well as, activity reports pursuant to department activity protocols;

  • Attends program, departmental agency meetings, and performs other duties as assigned by supervisor and/or agency policies and procedures.

 

General Duties:

  • Adheres to Agency, Department and Team policies and procedures.

  • Participates in Agency, Department and Team meetings and events.

  • Performs other duties as assigned by supervisor or agency.

 

Supervision:

  • Director of My House & Gay Men's Prevention & Education Services at Nashville CARES

Education, Knowledge, Skills & Experience:

  •   Bachelor’s degree in Health/Wellness field preferred.

  •   Personal history as consumer of services for YAAMSM populations preferred.

  •   Experience with and knowledge of HIV/AIDS, social services, and the GLBT community preferred.

  •   Licensed driver with valid collision & liability insurance and clean DMV record.

 

Qualifications: Ability to perform at the level of someone with a bachelor's degree in relevant field. Candidates should be able to effectively address issues specific to homosexuality, the coming out process, HIV related stigmas, etc. Experience in at least two of the following:  peer education outreach, group facilitation, volunteer coordination or support, and and/or community organizing/mobilization preferred. Strong written, oral communication, public speaking and marketing skills a plus. Ability to work 40 hours a week and travel evenings, some holidays and weekends is essential.  A valid driver’s license, clean DMV record, and automobile insurance are required.

Many Men, Many Voices: 3MV Facilitator

 at MY HOUSE / Street Works

Full time Hourly (Non-Exempt) |  9/28/2020

 

QUALIFICATIONS:

  • Ability to perform at the level of someone with a bachelor's degree in relevant field.

  • Personal history as consumer of services for African American Men who have Sex with Men populations preferred.

  • Able to effectively address issues specific to homosexuality, the coming out process, HIV related stigmas, etc.

  • Peer education outreach, group facilitation, volunteer coordination or support, and and/or community organizing/mobilization preferred.

  • Strong written, oral communication, public speaking and marketing skills a plus. 

 

RESPONSIBILITIES:

  • To provide consistent leadership and programmed facilitation for a series of curriculum based workshops.

  • To encourage group participants to share and express their concerns, fears, and feelings with each other.

  • To distribute informational handouts to group participants.

  • To work cooperatively with the other facilitators in providing group facilitation.

  • To complete all record keeping requirements in an accurate and timely manner.

  • To notify coordinator of group level interventions of an inability to facilitate a workshop session at least 48 hours in advance.

  • Completes activity reports and documentation in accordance with department processes;

  • Reports on output and outcomes, as well as, activity reports pursuant to department activity protocols;

  • Attends program, departmental agency meetings, and performs other duties as assigned by supervisor and/or agency policies and procedures.

 

GENERAL DUTIES:

  • Adheres to agency, department and team policies and procedures;

  • Participates in agency, department and team meetings, events, and strategic initiatives;

  • Participates in monthly individual administrative supervision;

  • Engages in planned, as well as organic, activities to actively promote counseling services throughout the community;

  • Meets and/or exceeds established productivity measures; 

  • Performs other duties as assigned by supervisor or agency.

 

SUPERVISION:

  • Prevention Director at Street Works and Director at My House

  • The 3MV workshop facilitator will be supervised by the 3MV Coordinator at My House.

  • Monthly supervision and team meetings will also be expected with the My House Project Director and program staff.

  • All reports and documentation are turned in monthly to the lead agency in the Quality Assurance office at My House facility.

 

EVALUATION:  

  • The 3MV workshop facilitator will be evaluated by the 3MV Coordinator at My House along within put from the My House Project Director.

  • Evaluation criteria will be fulfillment of job responsibilities.

 

Salary Range: $28,000 -$32,500
